According to a British Medical Journal Quality and Safety report, of over 1000 UK doctors and 1891 US doctors questioned, 19% of them have in the past three years had direct experience of an incompetent or poorly performing health care professional. Worryingly, of those, one in three decided to keep quiet as they were frightened of speaking out. Fear of retribution was the most common reason given for remaining silent. One in four chose not to speak out because they thought someone else would do so, but one seventh of those who did not speak out felt that nothing would be done about the incompetent professional even if they did speak out.

Baby death 'a preventable tragedy'
The death of a baby after a series of errors by a hospital was "a preventable tragedy", a solicitor has said.
Sarah and Scott Wallace have been awarded compensation after their son Harry died just hours after his birth at Scarborough District General Hospital in September 2009.
Their claim for medical negligence was settled with the Scarborough and North East Yorkshire Healthcare NHS Trust after an independent report highlighted a number of mistakes made after Harry's birth.
The report, commissioned by Neil Hudgell Solicitors, found that staff at the hospital failed to follow the proper respiratory support procedures; failed to transfer Harry to a special unit in Leeds; and failed to ventilate Harry and to help him breathe by properly putting a tube in his windpipe as his condition deteriorated.
Mrs Wallace, 29, also claimed that she was not allowed to visit her baby in the special care unit during the night because staff shortages meant there was no-one available to take her.
